I think the saying is more about not choosing a book based on the lovely color the cover is, or whether or not there's a pretty picture on the front. Or, for people, don't assume someone is scum just because they aren't dressed or made up the way you would want, and don't expect that everyone that's attractive and dresses snazzy to be awesome.
